The Volley 123 application form enables your club to register on the Volley 123 process. The aim of the application form is to give both your club and the National Club Development Officer an idea of where your club is at the minute.

The form can be completed electronically (please right click the link above and select Save Target As... to save a blank copy of the application form) and e-mailed to clubs@volleyballengland.org or a hard copy can be printed (or sent on request) and should be returned to: National Club Development Officer, Volleyball England, Suite B, Loughborough Technology Centre, Epinal Way, Loughborough, LE11 3GE.

The application form covers briefly the four modules that are involved in creating a quality club:

• Club Management
  
• Playing Programme
  
• Duty of Care
  
• Sports Equity & Ethics

To complete the form simply answer YES or NO to each question and give a brief explanation if it asks for one. There are 3 questions in each key area within each module. Once you have answered every question fill in the ‘score’ section for each ‘Key Area’ with a score of 0-3 depending on how many ‘Yes’ answers you have.

You can now see which areas you are strong in and which areas that your club can look to improve on.
